ALL-SPORT & SEASON TICKET ORDER FORM
2011-2012 

Please use the form on the reverse side of this sheet to order All-Sport tickets as well as Season Football & Season Basketball Tickets for the 2011-12 school year: Instructions for completion of the form are listed below:

  1. Please use one form per family Click Here for PDF of Ticket Form

  2. Next to the name, please place an "X" under the type of ticket you would like to order for that person. There should be one "X" per line.

  3. Fill in the “Total Enclosed” blank above the contact information box.  Checks may be made payable to EMHSAD.  Payment is due when order is submitted. (Cash or checks only)

  4. Before filling in the “Seating Questionnaire” box with the appropriate information, please note the following:

    • All-Sport ticket holders who have their forms in by July 25th will receive their seating assignments before season ticket holders. This means that a person who buys an All-Sports ticket could sit in a seat that has been occupied by a season ticket holder in the past.

    • For those who want the same seats as last year, please indicate on your form where you were sitting and every attempt will be made to sit you as close as possible.

    • If you wish to sit with other All-Sport ticket holders, please indicate that on the appropriate lines.  Every effort will be made if the forms are submitted together.

    • Ticket prices are as follows:

Pre-school children will be admitted free as long as they sit in their parents’ laps.  If a seat is needed then you will need to purchase a ticket.

Tickets may be picked up in the Athletic office beginning August 8th. They may also be picked up at the first home game as long as they have been ordered and paid for.  They may also be picked up at the first home game as long as they have been ordered and paid for.

Ticket order forms need to be in the athletic office by July 25th in order to retain seats from previous year.

Enclosed is your 2011-2012 All Sport Ticket(s) form.  This card will admit you to all Memorial High School home sport events except IHSAA and NLC tournaments and Charger Invites.  Please safeguard these cards as you would cash.  Lost cards may be replaced according to the following fee schedule.

IMPORTANT

Memorial High School students who purchase an all-sport ticket will need to come to the athletic office first and then go to the student office to have their all-sport status put on their ID. They will need to have their ID to get into any sports events or they will be charged. If they lose their ID card they will be charged $5.00 to add the all-sport status along with the charge to replace the ID. ALL HIGH SCHOOL STUDENTS WILL BE CHARGED TO GET IN THE GAME IF THEY DO NOT HAVE THE ALL-SPORT STATUS ON THEIR ID. THEY WILL NOT BE ALLOWED IN WITH AN ADULT OR CHILD ALL-SPORT CARD.
